What I Do as a Senior Transision Specialist

As a Certified Senior Transition Specialist, my role is to act as your family’s guide and advocate. I don’t just look at a house — I look at the bigger picture of what’s best for your loved one.

Here are ways I can help:

  • When a loved one’s health changes, families often face urgent and emotional decisions. I’m here to guide that transition with clarity, speed, and compassion.
  • I connect families with trusted local partners—like movers, estate planners, and senior care providers—to make the process smoother and more manageable.
  • I simplify complex transitions by coordinating logistics and reducing stress, so families can focus on what matters most: their loved one’s wellbeing.
  • In many cases, the home is a key asset that can help fund the next chapter. I offer clear, fair options for transitioning out of the home—whether through traditional listing or an as-is sale.

About Tom

Certified Senior Transition Specialist &

Family-Focused Problem Solver

Hi, I'm Tom Welch. I’ve spent most of my life in the heart of Iowa—growing up in Urbandale, raising my family in Grimes, and building relationships across Johnston, Clive, Adel, and Dallas Center. This community isn’t just where I live—it’s where I serve.

Before launching Turn to Tom, I spent over two decades in healthcare IT leadership, helping hospitals and care teams solve complex problems with clarity and compassion. That experience taught me how to navigate systems, advocate for families, and always keep the human side of care front and center.

Now, as a Certified Senior Transition Specialist, I bring that same mindset to families facing one of life’s hardest problems - what to do when a loved one can no longer live at home. I help families solve that problem by dealing with the home, helping locate the best community for their loved one, and providing overall guidance and clarity on the transition process so families can focus on what matters most.

I’m also a husband, a dad, and a proud member of the Grimes pickleball league (still chasing that first win!).

I know what it means to care deeply for family, and I treat every client’s story with the same respect and urgency I’d want for my own.

When it’s time for change, you don’t have to figure it out alone. Turn to someone who knows the community, understands the healthcare landscape, and puts families first.

You don't have to figure this out alone.

If your family is facing a transition, or if you’re a professional supporting families through this season of life, I’d be honored to help. Let’s talk about your options.
